This Liability Waiver and Release of Claims (“Agreement”) is entered into between the participant {name} (or parent/guardian, if participant is a minor) and Grappling Disciples Jiu Jitsu LLC, including its owners, instructors, employees, contractors, volunteers, representatives, and affiliates (collectively “the Academy”).

By signing this Agreement, the participant acknowledges and agrees to the following:

1. ASSUMPTION OF RISK

I understand that Brazilian Jiu-Jitsu (“BJJ”), mixed martial arts, self-defense training, strength and conditioning, open mat participation, sparring, drills, and any other activities offered by the Academy involve inherent risks, including but not limited to:

• Strains, sprains, and muscle injuries

• Broken bones

• Joint injuries (including shoulders, knees, elbows, fingers, etc.)

• Chokes, joint locks, and submission techniques

• Bruising, cuts, concussions, and contact injuries

• Collisions with mats, equipment, or other participants

• Transmission of illnesses or skin conditions

• Serious injury or death

I voluntarily choose to participate with full knowledge of these risks and accept full responsibility for my participation.

2. RELEASE OF LIABILITY

In consideration for being allowed to participate in activities at the Academy, I hereby release, waive, and discharge the Academy from any and all liability, claims, demands, actions, or causes of action arising from:

• Personal injury

• Property damage

• Wrongful death

• Negligence (excluding gross negligence or intentional misconduct, to the extent prohibited by North Carolina law)

This release applies to incidents occurring on the premises, during off-site events, or in connection with any Academy-sponsored activity.

3. MEDICAL FITNESS & AUTHORIZATION

I certify that I am physically able to participate in BJJ and related activities. I agree to:

• Inform instructors of any medical conditions or limitations

• Stop training immediately if I feel lightheaded, faint, or injured

• Allow emergency medical care to be administered if deemed necessary

I understand that I am solely responsible for any medical expenses incurred as a result of participation.

4. RULES & BEHAVIOR

I agree to follow all Academy rules, instructor instructions, and safety guidelines, including:

• Using proper hygiene and clean training gear

• Showing respect to instructors and training partners

• Avoiding reckless or dangerous behavior

• Tapping early to submissions

The Academy reserves the right to revoke training privileges for unsafe or inappropriate conduct.

5. MEDIA RELEASE (Optional)

I do / do not consent to the Academy using photos or videos of me (or my child) for marketing, social media, or promotional purposes.

6. MINOR PARTICIPATION (If Applicable)

If the participant is a minor:

I, the parent or legal guardian, understand the risks and consent to the minor’s participation. I agree to all terms in this Agreement on behalf of the minor and myself.

7. GOVERNING LAW

This Agreement shall be governed by the laws of the State of North Carolina. If any provision is found unenforceable, the remaining provisions shall remain in full force.
